Maybe you just learned that your child has special heath care needs, or maybe you have been trying to help your child for a long time. Maybe your child is a newborn or maybe your child is a teenager. Either way, HCP will stand by you. HCP assists children with special health care needs, from birth to age 21, who live in Chaffee County.
HCP serves children from birth to age 21. There are no income requirements
• Care Coordination is available for children with complex health needs. (These children are referred after birth and ARE followed intermittently to assess if they require further intervention or services. CCPH nurses assist in coordinating services for the child. Newborn hearing screens offered. Referral of NICU clients.)
• Pediatric Medical Specialty Clinics which include:
Cardiology (heart, vascular system)
Neurology (nervous system, brain-to-muscle connections)
Orthopedics (bone, joint, ligament and muscle functions)
Otolaryngology (ears, nose and throat)
Pediatrics (children’s medicine)
Rehabilitation (training or therapy to return to normal functioning)
Genetics: Dr. David Manchester, Geneticist.
All children ages 0-21, with or without a payer source, are welcome to attend these clinics. However, blood tests, procedures, and medications are not covered.
• PartC/Early Childhood Connections partners with HCP to assure that medical and educational services are provided as needed for children birth-3 years of age.
• Traumatic Brain Injury (TBI): The Brain Injury Association of Colorado
(BIAC) contracts with HCP to provide extensive care coordination for eligible children with TBI, ages 0-21. Services include education, access and financial assistance for children of TBI to meet their full potential
• Transition to Adult Life: Youth with special needs are supported into adult life,
Beginning at age 14, through appropriate health care, education, vocational and independent living opportunities. Parents of children with complex medical needs are encouraged to apply prior to the child’s 18th birthday to retain guardianship.
